Partner Referral Guide for At-Home Wound Care
Preferred Healthcare provides advanced wound care to patients in homes and residential care settings across the Atlanta area. Our mobile wound care team works closely with referring partners to deliver accurate assessment and responsive treatment. We also maintain clear documentation throughout the healing process. When patients need ongoing treatment outside of a traditional wound care setting, we bring expert wound management directly to them.

Is Mobile Wound Care the Right Fit for Your Patient?
Our mobile wound care program supports patients with acute or chronic open wounds that require ongoing provider oversight. We deliver advanced wound management directly to a patient’s residence or bedside, ensuring access to specialty care for homebound individuals and those with limited ability to travel to a wound clinic.
Patients with complex or delayed-healing wounds often require expert onsite evaluation and continued clinical management. And when advanced interventions such as debridement or bioengineered skin substitutes are needed, referral to mobile wound care can help optimize healing outcomes.
Types of Wounds We Commonly Treat
Our wound specialists treat a wide range of wound types at all stages of healing. Whether managing acute injuries or chronic non-healing wounds, we develop a treatment plan based on each patient’s needs. Here are some of the common types of wounds we treat.
- Pressure injuries
- Diabetic foot ulcers
- Arterial and venous ulcers
- Post-operative wound complications
- Surgical wound dehiscence
- Traumatic wounds
- Burns appropriate for outpatient management
- Wounds with necrotic or non-viable tissue
- Wounds requiring negative pressure wound therapy
You can refer a patient to us for evaluation and wound care at healthcare facilities across the Atlanta area. We visit a range of clinical settings including assisted living, home health and palliative care, hospitals, memory care, long-term care, primary care clinics and skilled nursing facilities.
When Mobile Wound Care May Not Be Appropriate
Not all referrals will be appropriate for mobile wound care. We are unable to accept patients whose goals are exclusively palliative and without active healing objectives, and those with closed wounds, including suspected deep tissue pressure injuries. Patients with carcinoma wounds requiring symptom management rather than wound-directed care are also outside the scope of our mobile wound care services.
What to Expect After a Patient Referral
Our partner referral process is straightforward and prompt, designed to get patients the care they need quickly. Once the referral form is completed, you can expect these steps.
- Referral Confirmation – We promptly acknowledge receipt of your online referral and begin coordinating next steps.
- Clinical Review – Our clinical team evaluates the referral documentation to determine eligibility and level of care needed.
- Wound Care Planning – An individualized treatment plan with visit frequency is established based on clinical requirements and insurance guidelines.
- Ongoing Communication – Referring partners receive consistent updates and documentation throughout the course of care.
For patients with hard-to-heal wounds, onsite provider-led wound management can offer convenience and improved healing progress. With advanced wound care delivered at the bedside, it’s possible to respond effectively to patient needs and reduce wound-related hospitalizations.
