You should refer any patient with an acute wound, a non-healing wound > 30 days duration, a diabetic wound, a pressure wound, a traumatic wound, a surgical wound, those being treated with negative pressure wound therapy (i.e., VAC), or those being treated in an SNF with a stage 2 wound or greater.
